What is Basivertebral Nerve Ablation?
Basivertebral nerve ablation (BVN ablation) is an innovative, minimally invasive procedure that targets the basivertebral nerveāthe nerve located within the vertebral body that transmits pain signals from damaged vertebral endplates. This treatment is specifically designed for patients with chronic low back pain originating from the vertebrae themselves, known as vertebrogenic pain.
Unlike traditional treatments that address surrounding structures like discs or facet joints, BVN ablation targets the source of pain within the bone itself. The procedure uses radiofrequency energy to ablate (heat and disable) the basivertebral nerve, interrupting pain signal transmission from damaged vertebral endplates.
Who is a Candidate?
BVN ablation may be appropriate for patients who:
- Have chronic low back pain lasting 6 months or longer
- Have not responded adequately to conservative treatments
- Show Modic changes (Type 1 or Type 2) on MRI
- Have pain originating from vertebral endplates
- Are not candidates for or wish to avoid spinal fusion
- Have disc degeneration without significant nerve compression
The Procedure
Preparation
The procedure is performed under fluoroscopic (X-ray) guidance. You'll receive sedation for comfort and local anesthesia at the entry site.
Needle Placement
A specialized introducer needle is guided through the pedicle into the vertebral body, positioning near the basivertebral nerve.
Ablation
Radiofrequency energy is delivered to heat and ablate the basivertebral nerve, typically treating multiple vertebral levels.
Recovery
The procedure takes approximately 1-2 hours. Most patients go home the same day with activity restrictions for a few weeks.
Benefits of BVN Ablation
Long-Lasting Relief
Clinical studies show sustained pain relief for 5+ years in many patients.
Minimally Invasive
No major incisions, hardware, or spinal fusion required.
Preserves Mobility
Unlike fusion, maintains natural spinal movement and flexibility.
Quick Recovery
Most patients return to normal activities within 2-4 weeks.
What to Expect After Treatment
Recovery from BVN ablation is typically straightforward. You may experience some soreness at the treatment site for a few days. Pain relief usually develops gradually over 2-6 weeks as inflammation subsides. Most patients can return to light activities within a week and resume normal activities within 2-4 weeks.
